Your Medicare Annual Health Assessment (AHA) is a great way to take charge of your health by focusing on prevention and learning what steps you should be taking to get and stay healthy.
Staying on track with assessment, however, can sometimes feel challenging. We’re here to help make it as easy as possible by offering several in-person health clinics throughout the Bay Area. If you’re fully vaccinated against COVID-19, simply choose a convenient time and location from the list below and call us to make an appointment.

Please be sure to bring your proof of insurance as well as your ID. Proof of COVID-19 vaccination and booster may be required. To comply with social distancing requirements, we kindly ask that you only bring 1 person with you (if necessary).
You may also be eligible to receive your assessment in your home or through a virtual visit. When you call to make an appointment, be sure to ask the Brown & Toland representative about your options.
